Description The Nurse navigator's primary function is to seamlessly guide the patient and family through the health care system. Activities include providing disease-specific education and information, referral navigation, sharing knowledge in regard to treatment options, expediting patient access to diagnostic and treatment procedures, connecting patients with appropriate resources, providing ongoing support to patient and family, responding to patient/family needs and addressing issues of access to care as part of the navigation team. Collaborates with social work, patient financial counselor, chaplains, dietitian, primary care, and volunteers to provide comprehensive navigation services to patients, families and the community at large. The nurse navigator works in collaboration with the physicians, patient, and patients' family, and members of the health care team. The RN Navigator assesses, plans, implements, evaluates, and coordinates treatment plans. Montana Registered Nurse License.
