Job Summary We are seeking a dedicated and compassionate Skilled Nurse in Home to provide exceptional patient care in a home setting. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs. Administer medications via injections, IV infusions, and other routes while adhering to strict safety protocols and HIPAA regulations. Manage complex medical procedures such as catheterization, tube feeding, wound suturing, and ventilator care in accordance with established clinical guidelines. Conduct diagnostic evaluations using medical imaging techniques when necessary and coordinate with physicians for treatment planning. Document all patient interactions accurately utilizing EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems like Visiting Aid Provide specialized care for patients with Alzheimer’s disease, dementia, developmental disabilities, or those requiring memory care and behavioral health support. Collaborate with case management teams to develop discharge plans, coordinate post-acute care services, and facilitate smooth transitions across healthcare settings. Educate patients and their families on medication administration, pain management strategies, nutrition, and home safety to promote independence and well-being. Experience Proven experience working as a nurse in diverse settings such as home care, hospital medicine, ICU (Intensive Care Unit), emergency medicine, or long-term care facilities. Hands-on expertise with IV insertion, phlebotomy, sterile processing techniques, and infection control practices. Familiarity with ICD-9/10 coding systems for accurate medical documentation and billing processes. Experience working with individuals with disabilities or special needs such as developmental disabilities or neonatal ventilator management is highly valued. Knowledge of anatomy, physiology, pharmacology related to pain management and acute care procedures. Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks to ensure precise recordkeeping and patient data management. Ability to perform physical examinations and assessments confidently while demonstrating excellent communication skills.
