Baywood Court is actively recruiting for a Social Services Designee in our Health Center (Skilled Nursing Facility). The Social Services Designee plays a vital role in supporting residents and their families by addressing psychosocial needs, easing transitions into the care environment, and ensuring thoughtful, well‑coordinated discharge planning. This position serves as a trusted advocate and resource, helping residents maintain dignity, well‑being, and continuity of care throughout their stay. Working collaboratively with residents, families, and the interdisciplinary care team, the Social Services Designee contributes to person‑centered care planning and problem‑solving while navigating complex emotional, social, and regulatory considerations. Hourly pay range: $30 - $35; Mostly Monday to Friday We are a 24/7 facility therefore one must be able to work weekends and/or holidays as required. All applicants are required to complete and pass a drug screening, physical exam with tuberculosis (TB) skin test, and criminal clearance through FBI and Department of Justice. Participate in interdisciplinary care plan meetings and contribute to individualized care plans. Identify psychosocial, emotional, and discharge planning needs of residents. Provide counseling, emotional support, and crisis intervention to residents and families. Facilitate communication between residents, families, and facility staff. Address resident concerns and grievances in accordance with facility policies. Coordinate safe and timely discharge planning, including referrals to community resources. Facilitate transitions of care to hospitals, home health, assisted living, or other settings. Connect residents and families with community resources such as financial assistance, transportation, and mental health services. Advocate for residents’ rights and ensure respect, dignity, and self-determination. Ensure compliance with federal (CMS), California Department of Public Health (CDPH), and Title 22 regulations governing social services. Bachelor’s degree in Social Work or a related human services field preferred OR Licensed Social Worker (LCSW) or Associate Clinical Social Worker (ACSW) (not required but preferred) OR High school diploma (or equivalent) and at least 5 year of su
