Job Description
Summary The Long Beach VA Health Care System is recruiting for (1) Social Worker (Community Living Center) (Acute Rehabilitation Unit). The incumbent provides Veterans with a range of social work services, including psychosocial assessment, work with families and caregivers, short term case management, discharge planning, advocacy and service coordination, and linkage with VHA and non-VHA community services. The duties are accomplished in concert with other members of the Inter-Disciplinary Team (IDT). Duties VA Careers - Social Work : https://youtube.com/embed/enRhz\_ua\_UU Duties may include, but are not limited to: Completes treatment plans and collaborates with the multidisciplinary team to establish achievable goals with the Veteran/family Acts as an advocate when it serves in the best interest of the Veteran and family Provides education related to VHA-LBHCS and community resources, entitlements, Advance Directives/Living Will and will refer Veteran/family to the appropriate IDT for identified health education needs Reviews the progress notes from other providers to accurately determine the strengths and limitations of each Veteran referred for a Social Work assessment and interventions Provide Veterans and their family/Caregivers with ongoing supportive counseling Continually screens and assess Veterans for suicide by completing a Suicide Screen and/or the Columbia-Suicide Severity Rating Scale (C-SSRS) Establishes and maintains positive working relationships with employees, volunteers, consumers, and stakeholders with VHA and outside community agencies Attends all appropriate huddles and treatment team meetings, Social Work Service (SWS) All Staff Community Meetings, SWS Section meetings, and other committee meetings as appropriate Collaborates performance improvement processes and comply with performance measures as required by VHA for their specific population and or program Work Schedule : Full-Time; Monday-Friday; 7:30am-4:00pm Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
