Maintains performance improvement activities within the department. Education, Licensure, Registration and Experience: LSW OR MSW. Minimal two years experience in Social Work. Licensed in state LSW or MSW. Essential Functions Assures that social service/discharge planning activities are integrated and coordinated with the hospital/LTC system by effective communication at all levels.
