Join a growing home health team in Oak Brook, IL as a Speech-Language Pathologist providing patient-centered care in home settings. Deliver evaluations, develop Plans of Care, implement therapeutic interventions, and provide caregiver education while collaborating with interdisciplinary teams. A recruiter will reach out with more details. Minimum Requirements Master's Degree in Speech-Language Pathology from an accredited program. Certificate of Clinical Competence (CCC) in Speech-Language Pathology from ASHA, or CCC-eligible with intent to obtain CCC. (Required) Active Illinois state license to practice speech-language pathology. (Required) Proof of current CPR certification. (Required) Current driver's license, reliable transportation, and personal auto insurance. (Required) Minimum 1 year of clinical experience in Speech-Language Pathology. (Required) Experience in a home health or home care environment. (Preferred) Demonstrated ability to perform patient evaluations, develop and document Plans of Care, and provide caregiver education. Ability to perform physical tasks required for home visits, including lifting, bending, and prolonged standing/walking. HCHB, OASIS, and home health electronic documentation experience. (Required) Ability to work a flexible schedule and travel locally. Float to other local facilities may be required. Assignment Details Facility Type: Home Health. Position Type: Per Diem / Temporary Contractor, travel assignment. Start/End: Assignment window spans May through August (dates provided by client). On-call as needed per branch request. On-call needs are subject to change based on staff and branch needs. Expected weekly productivity: 30 units (approx. 6 units per week billed as noted by client).
