Powerback Rehabilitation is seeking a dedicated Speech Therapist-CFY to join our Healthcare & Medical Services team, specializing in post-hospital rehabilitation and recovery care. In this role, you will work under the supervision of an experienced, ASHA-certified Speech-Language Pathologist to provide evidence-based speech, language, cognitive-communication, and swallowing services to adults recovering from surgery, injury, or illness. You will perform comprehensive evaluations, administer standardized and functional assessments, and develop individualized treatment plans that support patients in regaining independence and improving their quality of life. Working closely with an interdisciplinary team of physical therapists, occupational therapists, nurses, and physicians, you will contribute to coordinated care plans, participate in care conferences, and communicate treatment goals and progress to patients, families, and caregivers with clarity and compassion. Daily responsibilities include delivering one-on-one and small-group therapy sessions, documenting care in the electronic medical record in accordance with regulatory and payer standards, monitoring patient progress, and adjusting interventions in collaboration with your supervising SLP. You will educate patients and families on strategies for communication, cognition, and safe swallowing, and provide training on the use of adaptive equipment or augmentative and alternative communication tools as appropriate.
