Hegira Health, Inc. (HHI), a private non-profit corporation, is one of the largest freestanding behavioral healthcare agencies in the area. HHI, accredited by the Joint Commission and licensed by the State of Michigan, provides a broad array of mental health and substance abuse treatment and prevention services to individuals of all ages. Oakdale Recovery Center is a 55-bed treatment facility for adults with substance use disorders. Oakdale Recovery Center is dedicated to assisting persons with substance use and co-occurring mental health and substance use disorders to begin the process of recovery and to regain control over their lives. Oakdale’s Detoxification and Residential programs provide intensive, structured, 24-hour medically monitored services for adults struggling with a wide range of drug, alcohol and mental health concerns. Oakdale Recovery Center provides a disease-specific, person-centered and multidisciplinary approach to care. We offer comprehensive treatment services and offer each resident follow along supports that begin with the withdrawal/detoxification process and extend through aftercare following residential treatment. Work Schedule PART-TIME: Day Shift: Friday, Saturday or Sunday, one day in the week EDUCATION AND EXPERIENCE REQUIREMENTS: Bachelor’s Degree, specializing in social sciences, psychology, social work, or counseling. CADC or a Development Plan approved by MCBAP obtained within 30 days of employment Provided direct treatment services to adult’s in a behavioral health impatient or outpatient setting for not less than 12 months, or successfully completed an internship as part of bachelor’s program including some experience working with adults with SUDs . Demonstrate basic knowledge of diagnostics ,psychopharmacology, and supportive treatment approaches as applied to the SUD adult population. Demonstrate knowledge of the identification and treatment of co-occurring mental health and substance abuse disorders. SUD Counselor RESPONSIBILITES: Intake Completion of the Case Management Assessment within 72 hours of intake appointment. Make recommendations regarding need for ongoing, targeted case management services. Identify issues that may exist regarding safety (personal and environmental), natural supports, and community involvement. Identify referrals needed for comprehensive treatment, including medical, nutrition, substance abuse, STD/HIV risk, education, and vocational training. Educate patient and, if applicable, patient guardian with respect to admission information packet, including but not limited to, Recipient Rights process. Schedule appointment for psychiatric evaluation within appropriate timeframe relative to medication supplies and/or acuity of symptoms. Treatment Planning and Progress Reviews Complete individualized Treatment Plan Case Management goals and objectives for each patient by the second session or within 30 days of referral for case management services (whichever occurs first). Case Management goals and objectives on the Treatment Plan demonstrate that the plan for treatment was developed with the patient. Treatment Plan problem areas, goals, and objectives address the patient’s DSM five-axis diagnosis and areas identified on the bio-psychosocial assessment, including but not limited to, personal and environmental safety, relapse potential, child and family strengths, and individual needs. Discharge Process Document termination of case management services on the Treatment Plan and on a Progress Note when patient continues to receive other clinical services from HHI. Document on Treatment Plan criteria and anticipated date for planned discharge. Transports patients as needed to community-based supportive, adjunctive services. Interpersonal: Establish effective therapeutic bonds with patients assigned to caseload. Maintain collegial, cooperative relationships with peer therapists and other staff of HHI. Report all Recipient Rights complaints and knowledge of potential complaints to clinical supervisor within 24 hours of said knowledge. Contribute pertinent clinical information at regular case conference meetings.
