LOCATION: Connecticut OVERVIEW: The Love146 Survivor Care Transitional Services Specialist is a full-time position. The Survivor Care Transitional Services Specialist supports participants ages 16 and above in the Connecticut Survivor Care Program to achieve their goals as they prepare to transition into adulthood.
