Job Summary Cuready Healthcare Staffing is seeking a qualified Physical Therapist to support patient care and clinical operations within home health settings. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards. Responsibilities Conduct assessments and develop individualized care plans for patients. Provide physical therapy interventions in a home health setting. Document patient progress using EPIC/Oasis systems. Educate patients and families on home exercise programs and rehabilitation techniques. Travel within Medford, OR, and surrounding areas with minimal windshield time. Maintain compliance with facility policies and professional standards. Qualifications Hold a valid Physical Therapy license in the state of Oregon or eligible. Open to candidates with no prior home health experience. Basic Life Support (BLS) certification required. Reliable transportation for travel to patient homes. Experience with EPIC/Oasis charting systems preferred. Strong communication skills and ability to work independently.
