Shift: Days – 4x10s (8:00 AM – 6:30 PM) Duration: 13 weeks Job Duties: Provide social work services and discharge planning in an acute hospital setting. Assess patient psychosocial needs and coordinate appropriate care resources. Work with patients and families regarding Medi-Cal and Medicare services. Collaborate with interdisciplinary healthcare teams to support patient care plans. Document patient interactions and care coordination in Cerner EMR. Requirements • Active California LCSW license required Minimum 1 year of acute hospital care experience required 2 years of acute care experience preferred Knowledge of Medi-Cal and Medicare required Cerner experience strongly preferred Acute care hospital experience required RTO must be included at time of submission Start date, birthdate, and SSN required at submission
