Job Title:Travel LPN / LVN - Long Term Care (LTC) Location:Glenwood, Minnesota Shift:Day Shift - 40 Hours/WeekAssignment Length:9 WeeksWork Setting:OnsitePay Rate:$35/hour on W2 Position Summary We are seeking a dependable and compassionate LPN/LVN for a 9-week assignment in a Long-Term Care (LTC) and Rehabilitation setting.The ideal candidate will have prior LTC experience and the ability to provide quality patient care in a fast-paced healthcare environment.Key Responsibilities Provide direct nursing care to long-term care and rehabilitation patients Administer medications and treatments as prescribed Monitor and document vital signs and patient health status Perform blood glucose monitoring and related patient care Assist patients with activities of daily living while ensuring safety and comfort Maintain accurate documentation within EMR systems Collaborate with physicians, RNs, and interdisciplinary healthcare teams Float between facility units as needed Follow infection prevention and patient safety protocols Respond appropriately to changes in patient condition Required Qualifications Active LPN/LVN license required Minimum 1 year of LTC experience required Current BLS Certification required Experience in Skilled Nursing Facility or Nursing Home settings preferred Familiarity with EMR systems; Epic experience preferred Skills & Competencies Strong clinical and patient care skills in LTC settings Experience with blood glucose testing and monitoring Ability to manage higher patient ratios effectively Excellent communication, teamwork, and documentation skills Flexibility to float between units as needed For more details reach at vgill@navitassols.com or Call / Text at 516-862-1203..
