Assignment details Contract length: 13 weeks Shift length: 8 hour shifts Hours per week: 40 Start Date: ASAP Job responsibilities Educate patients on treatment plans, follow-up instructions, and self-management strategies. Conduct patient intake, focused assessments, and symptom screening aligned with clinic protocols. Perform medication reconciliation activities as directed and communicate discrepancies. Work 40 hours per week with 8-hour shifts.
