Assignment details Contract length: 13 weeks Shift length: 8 hour shifts Hours per week: 40 Start Date: ASAP Job responsibilities Perform medication reconciliation activities as directed and communicate discrepancies. Document care delivered, patient response, and communications in the medical record. Work 40 hours per week with 8-hour shifts. Conduct patient intake, focused assessments, and symptom screening aligned with clinic protocols.
