Assignment details Contract length: 13 weeks Shift length: 8 hour shifts Hours per week: 40 Start Date: ASAP Job responsibilities Support care coordination activities across providers to promote continuity of care. Document care delivered, patient response, and communications in the medical record. Educate patients on treatment plans, follow-up instructions, and self-management strategies. Work 40 hours per week with 8-hour shifts. Perform medication reconciliation activities as directed and communicate discrepancies.
