Assignment details Contract length: 5 weeks Shift length: 8 hour shifts Hours per week: 40 Start Date: ASAP Job responsibilities Educate patients on treatment plans, follow-up instructions, and self-management strategies. Conduct patient intake, focused assessments, and symptom screening aligned with clinic protocols. Work 40 hours per week with 8-hour shifts. Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams.
