Assignment details Contract length: 13 weeks Shift length: 7 hour shifts Hours per week: 37 Start Date: ASAP Job responsibilities Work 37 hours per week with 7-hour shifts. Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams. Support care coordination activities across providers to promote continuity of care. Communicate clinically relevant updates to providers to support timely decision-making. Document care delivered, patient response, and communications in the medical record.
