Job Summary We are seeking a friendly and compassionate travel nurse practitioner with experience in Neurology to join our team and support patients in the Patient Health Management (PHM) program. In this role, you will work closely with care coordinators across various providers and settings to promote improved health outcomes. You will facilitate patient assessments, assist with communication between facilities and patients, and ensure accurate documentation through our proprietary software system. You will receive regular support and guidance under the supervision of an offsite Collaborative Physician.
