Key Responsibilities Provide high-quality patient care to stabilize and monitor patients transitioning from critical care to less acute settings. Administer medications, perform assessments, and update care plans according to patient needs. Collaborate with multidisciplinary teams to ensure optimal patient outcomes. Document patient progress accurately in electronic health records. Educate patients and families on post-discharge care and recovery processes. Skills Needed Current RN license in New York.
