Assignment Details: Schedule: Monday – Friday Shift: 8:00 AM – 5:00 PM Weekend Rotation: Required (rotating weekends) Key Responsibilities: Evaluate and treat patients in their homes to improve daily living and functional skills Develop and implement individualized treatment plans Educate patients and families on therapy techniques and adaptive strategies Monitor patient progress and adjust care plans as needed Maintain accurate and timely documentation Requirements: Minimum 1 year of experience as an Occupational Therapist Prior Home Health experience required Active Illinois OT License BLS Certification (AHA preferred) Strong communication and organizational skills Ability to work independently and manage caseload effectively Additional Requirements: No local candidates within a 50-mile radius
