Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapy Assistant for a travel job in Stockton, Illinois. You will work with patients recovering from injuries or illness, helping them move better, feel better, and stay active under the guidance of a licensed Physical Therapist. Active Illinois PTA license required to start the assignment. BLS certification from AHA or ARC may be required. Experience working in a skilled nursing facility or long-term care setting preferred. Prior experience providing care to older adults and working with common SNF equipment (gait belts, Hoyer lifts, walkers) preferred. Ability to work collaboratively with interdisciplinary team and follow PT-directed plans of care. Assignment Details Facility Type: Skilled Nursing Facility. Shift Name: Days. Shift Details: Days 08:30–16:30. Schedule: Five 8-hour shifts per week. Floating: Candidate may be required to float to other facilities as needed.
