Details Home health setting serving a geriatric patient population Territory covers the Santa Cruz/Watsonville area, within a 50-mile radius of the hub/office Caseload of 5–7 patients per day Monday through Friday schedule, 8:30 AM – 5:00 PM with a 30-minute lunch break No on-call or weekend requirements Mileage between patient homes is reimbursed at the IRS standard rate EMR: HCHB (experience required) Requirements Minimum 1 year of home health physical therapy experience required Active California PT license in hand required (no pending licenses accepted); license required if applicable Current BLS certification required Experience with HCHB EMR required OASIS experience required Additional Information First-time travelers will not be considered for this position Local candidates are accepted at the travel rate Two professional references from a supervisor within the past 2 years required for consideration All time-off requests must be submitted at the time of initial consideration American Traveler Job ID #P-704622.
