Minimum Requirements Associate's, Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. (Client minimum: Associates Degree accepted.) Active Kentucky Physical Therapist license required to start the assignment. Home health experience required. Must be proficient with HCHB electronic documentation. Experience performing OASIS assessments, admissions, recertifications, 30-day assessments, and discharges. Experience supervising PTAs. BLS certification from AHA or ARC may be required. Preferred: prior experience working in Logan and Todd county coverage areas. Assignment Details Facility Type: Home Health Shift Name: Days Shift Schedule: Monday - Friday, approximate daytime schedule (am to 4:30 pm) as indicated by the client. Weekly Guarantee: 32 hours per week. Productivity Expectation: 30 points. Coverage Areas: Logan County and Todd County, KY. Mileage reimbursement between patient visits from first to last patient: Yes. On-call/Weekend Requirement: No. Dress Code / Scrub Color: Royal blue. Floating Requirement: Candidate may be required to cover assigned geographic areas as needed.
