ProLink Allied is seeking a travel Physical Therapist for a travel job in San Jose, California. Job Description & Requirements Specialty Physical Therapist Discipline Therapy Start Date 05/11/2026 Duration: 8 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel We are seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health and Hospice team serving patients in the East Bay Area. Clinician are needed in Salinas, Santa Cruz and Santa Clara. In this role, you'll work one-on-one with patients in the comfort of their homes to help them regain mobility, manage pain, and improve their quality of life. You’ll collaborate with a multidisciplinary team to develop and implement individualized care plans. Responsibilities • Conduct in-home physical therapy evaluations and assessments • Develop and implement individualized treatment plans based on patient needs and goals • Educate patients and caregivers on therapy exercises and safe mobility techniques • Monitor and document patient progress in accordance with agency and regulatory guidelines • Coordinate care with nurses, occupational therapists, physicians, and other healthcare professionals • Provide ongoing updates to the clinical team and revise treatment plans as needed • Ensure compliance with all state, federal, and agency policies and procedures ProLink Allied Job ID #D-171090.
