In this role, you will evaluate residents and develop individualized treatment plans to help improve mobility, reduce pain, and enhance overall quality of life. Responsibilities: Conduct patient evaluations and establish personalized treatment plans Provide therapeutic exercises, manual therapy, and mobility training Monitor patient progress and update care plans as needed Maintain accurate and timely documentation Educate patients, families, and caregivers on therapy programs Collaborate with interdisciplinary healthcare teams Requirements: Active Physical Therapist license in the state of Massachusetts Experience in long-term care preferred Strong clinical and communication skills Ability to work independently and manage a full caseload Commitment to high-quality, patient-centered care Why Join Us?
