Job Description & Requirements Specialty Physical Therapist Discipline Therapy Start Date 05/11/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Opportunity Position: Physical Therapist - Skilled Nursing The Physical Therapist collaborates with residents and healthcare providers to deliver physical therapy services aimed at enhancing residents’ functional abilities and independence in daily activities. Essential Duties and Responsibilities Promotes and supports the organization’s mission and values. Conducts evaluative and therapeutic procedures under physician orders while adhering to necessary precautions and guidelines. Evaluates residents and establishes a therapeutic plan of care with functional long- and short-term goals, specifying frequency, duration, and modalities of services. Incorporates input from residents and primary caregivers into the therapy goals. Consults with referring physicians and healthcare team members regarding the plan of care and discharge needs. Attends relevant team meetings. Monitors and assesses treatment effectiveness, modifying the treatment plan as necessary to meet established functional goals. Documents all evaluative and therapeutic interactions in the Electronic Medical Record, ensuring clarity and accessibility for team members. Recommends and obtains appropriate assistive devices for residents, ensuring proper instruction in their usage. Provides education to residents and caregivers, documenting the results of teaching. Refers residents to Physical Therapist Assistants as appropriate and collaborates with other therapy professionals for optimal care. Ensures compliance with Medicare and reimbursement regulations through accurate documentation. Delivers in-service education on clinical knowledge to nursing and other departments while maintaining current skills through continuous learning. Assists in the development of new rehabilitation programs and services. Stays informed about professional issues through memberships and publications, sharing relevant information with staff. Supervises assistant-level staff as appropriate, ensuring compliance with regulations and performance standards. Mentors new staff during orientation, ensuring completion of necessary competencies. Maintains department cleanliness and follows infection control protocols. Holds active licenses, certifications, and competencies, completing compliance training as required. Performs other duties as assigned within this role. Job Requirements Knowledge and Skills Strong verbal, written, and presentation communication skills. Responsible and supportive team player with strong interpersonal skills. Analytical, possessing sound decision-making capabilities. Proficient in Microsoft Office. Excellent time management and documentation skills. Compassionate approach to resident-centered care. Ability to understand and follow written instructions in English. Education and Experience Bachelor’s degree required; Master’s or Doctorate preferred. Active Physical Therapist license in the applicable state. Basic Life Support (BLS) certification preferred. Prior experience in skilled nursing or long-term care preferred. Equivalent combination of education and experience may be considered. Working Conditions/Physical Demands Must possess the physical and mental abilities to perform tasks associated with this position, including sitting, standing, walking, bending, and lifting up to 50 pounds. Work may require lifting/carrying frequently up to 20 pounds with some stress due to the responsibility for quality resident care.
