Job Description
Position Title: Veterans Counselor/Therapist Supervisor: Clinical Supervisor About Us: Transitions, Inc. is a CARF-accredited behavioral health organization providing comprehensive care for individuals impacted by substance use and co-occurring mental health disorders. We specialize in evidence-based recovery services with a focus on veterans. Position Overview: The Veterans Counselor/Therapist manages a clinical caseload, delivering individual and group therapy, conducting assessments, and developing treatment plans for clients, including veterans. This role involves collaborating with a multidisciplinary team to support effective recovery. Key Responsibilities: Provide Therapy & Counseling: Lead individual and group therapy sessions for veterans and their families, fostering healing and recovery. Assessment & Treatment Planning: Conduct assessments, create and update treatment plans, and recommend appropriate care levels. Multidisciplinary Collaboration: Participate in weekly team meetings to discuss client progress and ensure cohesive treatment plans. Documentation & Compliance: Ensure timely, accurate documentation of clinical services in line with regulatory and agency standards. Supervision & Development: Engage in weekly supervision with a Clinical Supervisor to support professional growth and licensure. Crisis Intervention & Referrals: Provide crisis intervention as needed and make referrals to additional services when appropriate. Discharge Planning: Develop and implement discharge plans, including lower levels of care and housing referrals. Community Representation: Represent Transitions professionally in all interactions with community partners and referral sources. Qualifications: Master’s degree in Behavioral Health or a related field. Active Kentucky license (CSW, LCSW, LPCA, LPCC, LPA) and/or Certified Alcohol and Drug Counselor (CADC). Experience working with veterans and individuals impacted by substance use and co-occurring mental health disorders.
