(RTI) is seeking a dedicated Waiver Case Manager – Transitional Specialist to support waiver case management services across multiple counties. We work with individuals integrated into the community, living in their own homes and in our residential settings. Our mission is to help people improve their quality of life by empowering them with knowledge, resources, and skills while treating them with dignity and respect. The Transitional Specialist provides person-centered case management services to individuals diagnosed with serious and persistent mental illness, substance use disorders, and co-occurring medical conditions who are eligible to receive services through waivered (CADI, CAC, BI, DD, etc) and non-waivered (Rule 185) programs. This role focuses on supporting program continuity across counties(Dakota, Hennepin, Ramsey) by assisting with client intakes, providing caseload coverage, and coordinating case management services during staff absences. This position requires in-person training at our main office for the first 60 days. Role Responsibilities: Complete client intakes for all counties( Dakota, Ramsey and Hennepin) served by the waiver case management program. Provide caseload coverage across counties to support service continuity when needed. Provide coverage during waiver case management out-of-office days , ensuring clients continue receiving services and support. Monitor, respond to, and coordinate incoming calls to the waiver case management coverage line. Coordinate client intakes and discharges within set parameters and timelines. Collaborate with professionals such as physicians, mental health providers, and interdisciplinary teams to support service planning and care coordination. Ensure financial authorizations and provider contracts are in place for services. Ensure services are documented in individual records so information is readily available, results of services can be determined, and record-keeping standards are met. Access available community and housing resources so that necessary and relevant resources match an individual’s needs and preferences. Provide crisis assessment and intervention so individuals receive the assistance they need to remain safe in their home and community. Required Qualification and Experience: The Waiver Case Manager shall be qualified under Minnesota law as follows: Be a graduate from an accredited four-year college with a major in social work, psychology, sociology or a closely related field. OR Be a graduate from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency. At least 1–2 years of professional experience providing waiver case management services. Willingness to travel within the Metro Area, for applicable mileage reimbursements, with a current driver’s license, clean record, and personal vehicle with insurance; and able to transport clients in that vehicle. Must not be actively receiving services from Residential Transitions Inc. and must not have received services within the last two years.
