Job Overview We are seeking a skilled and compassionate Wound Care Nurse to join our growing home health team. This position is flexible and based on patient referrals and assigned visits. The nurse will provide specialized wound care services to patients in their homes while collaborating with physicians and healthcare professionals to ensure high-quality patient care and optimal healing outcomes. This role is ideal for nurses experienced in wound management who are looking for flexibility and independence in a home health setting. Responsibilities Perform wound assessments and ongoing evaluations Provide wound care treatments, dressing changes, and infection prevention care Monitor healing progress and report changes in patient condition Develop and implement individualized care plans Educate patients and caregivers on wound care management and prevention Coordinate care with physicians, case managers, and other healthcare professionals Maintain accurate and timely clinical documentation Follow all agency policies, infection control standards, and home health regulations Qualifications Current California RN or LVN license in good standing Wound care experience preferred Home health experience is a plus Current CPR certification Strong clinical assessment and documentation skills Valid driver’s license and reliable transportation Ability to work independently and manage schedules efficiently Preferred Qualifications Wound Care Certification (WCC, CWCN, or similar) preferred but not required Experience with diabetic wounds, pressure ulcers, surgical wounds, and ostomy care Compensation Paid per visit / per client Competitive per-visit rates Flexible scheduling based on availability and patient assignments Schedule Flexible schedule Monday–Friday Weekend availability optional Work Location Patients are located throughout Los Angeles County and Orange County.
